The Frankfort Plant Board offers several types of long distance service for our customers. To change your current long distance service to FPB Long Distance, you will need to download, complete, and return to our Customer Service Department the appropriate application and authorization for service.
Below are links to the two types of service: Simple 6¢ A Minute Residential/Business Plan and Toll Free Service. Please select the appropriate link to download the appropriate application in Adobe PDF format.
Once downloaded, the application can be printed on your computer then filled out. Mail the printed application to: Frankfort Plant Board, Customer Service Department, P.O. Box 308, Frankfort, KY, 40602 or bring it to our 317 West Second Street address.
